Service in which visitation between family members (parent to child or child to child) is observed to ensure general safety and appropriate interaction is maintained during visitation. The Visit monitor either looks in on visits between family members periodically (as determined by DHS visitation plan) or observes the visit while sitting in the room with the family members. Visitation monitors will observe to ensure that no abuse, either physical or emotional, takes place during visitation. Monitor will also ensure that parents do not inappropriately try to influence the child to recant prior statements to CPS/police officers. This type of visitation is better suited for families where neglect was the primary type of abuse, and the child is not serving as the main witness/source of information to substantiate abuse/neglect. This type of visitation can be done at the family’s home, an outdoor recreational setting, or in an office, whichever provides the needed safety of the children and provider.
Supervised Visitation Two $90/hr
Service in which visitation between family members (parent to child or child to child) is observed to evaluate appropriateness and level of interaction. The observer/assessor will intervene if abuse or inappropriate behavior is occurring. The purpose is to assess and monitor the appropriateness of family reunification. The service must be identified on the service plan. Before each visit the provider will complete the following tasks:
• Meet with the child’s caseworker to learn the child’s needs as identified in the DHS/DYC Safety
plan and to jointly develop a specified needs list to be met during the initial visits.
• Call foster parents to obtain information related to the impact of separation on the child and the
child’s behaviors in order to refine the list of needs to be met during visits.
• Contact parent(s) to confirm the visit time and place, identify any barriers to visits and arrange to meet with the parent .25 hours before the visit.
• Notify the foster parent(s) of the planned visit and schedule transportation as needed.
• Meet with the parent before the visit to help the parent anticipate his/her own and the child’s
reactions during the visit and to discuss the needs to be met during the visit. (15 min, may be phone, zoom, or in person).
• Assist the parent as necessary during the visit.
• Meet with the parent after the visit to discuss how the parent met the child’s needs and to plan
changes in the next visit. (15 min, may be phone, zoom, or in person).
• Help the parent understand the importance of keeping his or her commitment to visit the child.
• Speak with the foster parent after the visit to discuss any behaviors or for the child reactions the child may display.
•Prepare notes about the parent’s skill in meeting the child’s needs during the visit and continuously refine the needs list.
In Home Family Preservation Services $120/hr [DHS Only]
“Eyes on” oversight required to provide structure and ensure performance of developmentally
appropriate activities necessary to carry out Activities of Daily Living and to ensure safety for the
identified client, family and/or community. The identified child or family requiring supervision must be within the defined boundary in which the provider can intervene immediately if needed to ensure safety, permanency and well being. This service is available at a dosage of 6-12 hrs per week, with a combination of in-home, virtual, and phone contact. Service may be available on an on call basis.
Life Skills $70/hr
Direct service in which the identified client is assisted to develop basic home management skills
and developing social/emotional support networks through hands-on implementation and role
modeling. This service provides for the acquisition of skills needed to meet adult role expectations and carry out Activities of Daily Living. Adult life skills are intended to improve the capacity for solving problems and resolving conflicts. Possible activities include food shopping, meal planning preparation, budgeting, utilizing community resources, accessing medical and school records, and attaining housing and or employment. This service is implemented when there is a lack of skill knowledge not due to a mental health condition and implies that there is not a lack of motivation. Provider will work with the client on the needs identified on the service plan.
Youth Mentoring $70/hr
Structured activities conducted under adult supervision that are designed to improve
the identified client’s functioning in the following areas: social skills, positive risk-taking behaviors, self-image, resiliency and self-management. Interventions should contribute to positive behavioral changes through role modeling, reinforcement and encouragement. The provider must link the youth with safe places, activities and/or programs to transition the youth to when service provision is complete. The service plan mandates the specific skills and activities that are needed. The staff to child ratio should be no greater than one to three for group activities. If multiple children are involved, they must all originate from the same family/case. The staff person must not be related to the family and is at least paraprofessional.
Case Management 70/hr
For an agency/independent provider to complete duties related to case management, including documentation, file maintenance, scheduling, and staffing with other professionals on the family team.The provider must be actively working with the client and submitting monthly summaries to the DHS worker/family court. Eligible for one representative per agency for Providers actively providing a treatment or safety service, for one hour per week per service.
Individualized Parent Education $120/hr
Direct face-to-face service to improve parental competence and knowledge of the following:
discipline, age-appropriate supervision, encouragement of child/adolescent care, age-appropriate development, and realistic expectations and standards of child/adolescent behavior of identified child. This service should be used to correct specific deficits in parenting. This service is provided in a one-on-one setting and is highly individualized to meet the parent’s needs. Specific examples include individualized behavior management techniques or understanding a child’s specific mental or physical health condition. Providers must use established curriculum or applicable parts of established curriculum. Providers are encouraged to use evidence/research-based and best or proven practice curricula or parts of such a curriculum that would be applicable for each client. Examples include but are not limited to:
Parent Coaching $70/hr
Individual service provided to clients who may need additional skills and support to manage unique parenting situations, including but not limited to families going through divorce/separation, (parents needing co parenting skills, parent child relationship coaching and support) foster parents (parents needing evidence based skills to parent challenging behaviors due to trauma and mental health diagnosis, and support to build relationships with children in their care) prospective adoptive parents (to support parenting through adoption) and kinship providers (family or family friends who are caring for children in their homes as result of out of home placement). This service will be provided by a trauma informed parenting professional trained in various evidence-based curriculum. This service may be provided via telehealth, in the home, or in the community.
